Travel Request Form (new form 6/26/08) - Questions go to Dora
"You do not have to print this form. Simply fill in the requested information. Click the Email Form button located at the bottom of the page and send to your supervisor for approval (you will have a copy in your sent folder for reference). The Unit Head will then forward the e-mail to me with approval notification (as an e-mail message), so that I may then get approval from Dana. If the supervisor does not approve the request then there is no need to forward it on to me. Unit Heads will simply complete the form and e-mail (as an attachment) to me. Once I have Dana’s decision, I will pass that along to you."
